I hereby grant the Hellinger Institute of DC all rights and permission to audio and/or videotape my participation in the workshop. I acknowledge that the workshop may be recorded in its entirety. Further, I understand and fully accept that these videotapes will be used for teaching purposes only with the trainees of the training program.
I understand that the issues addressed in family constellations may be of a highly personal and emotional nature and that I may experience or observe emotional or physical manifestations, accident or injury. I consider myself to have adequate mental, physical and emotional health to be able to accept all such risks, and I hereby agree to assume this risk, including but not limited to the types of responses and manifestations described herein.
I understand that confidentiality concerning all attendees and their issues presented in the group is expected and required. I hereby agree to refrain from discussing the content of the work outside of the workshop, except in such a way that each participant’s identity remains completely confidential.
I understand that this workshop is not designed as a substitute for therapy or any other professional consultation, but rather it is designed as an educational experience only. I understand that no guarantee of results have been implied or stated. I understand that family constellation work, though practiced for more than a decade in Germany and other countries around the world, is still considered innovative, and research into its long and short-term effects is ongoing.
All cancellation requests must be made in writing. No phone cancellations will be honored. A full refund, minus a $50 processing fee, will be made for requests postmarked 14 days prior to the workshop. No refunds will be made for requests postmarked within 14 days or less of the workshop date; however credit for the amount minus $50, can be applied to a future workshop. All refunds will be issued after the workshop.
